Ivan reveals how twisted he is.
Fernan finally meets his end.

Tony and Fernan drag new kids inside the warehouse. Joaquin and his men are watching over them. Dino asks if they will monitor Fernan all day long. Joaquin says he doesn't care, as long as he will find Junior.
In their dining hall, Verna tells her husband that Joaquin hasn't returned home. Tomas responds that her son is a policeman. Carmen walks by and Tomas calls her attention. He tells her how lucky she is to have Joaquin who's doing everything to save Junior.
Billy apologizes to his parents because he's inhibited from Junior's case. Norma asks if it was similar to Carmen's case. Billy promises to his parents they (cardo and him) won't stop looking for Junior. Edgar shares that Ryan is using the power of social media to search for Junior.
Two of Ivan's medical team ask what Fernan told him. They ask whether the police already know about them. He denies and says to feed Junior once he wakes up. Once they leave, Junior takes a peek. He pretended to be asleep all along.
Ivan is in his room drinking his pills. He tells himself there's no problem. Voices run in head saying he's a failure. This prompts him to drink another set of pills. He recalls how his mentor told him he was a failure. Ivan tries to suppress the memory and tells himself he saves children's lives. He tells himself he's not a failure.
Inside their car, Glen tells Cardo she has a hint that Belinda knows something, but is simply afraid to tell it. Cardo agrees that she might be afraid of the syndicate.
Belinda is sitting beside her son on his bed. She reads a news paper with a headline of a dead kid with a missing kidney. She opens the envelope Cardo gave her and browses through the files. She recalls what Cardo told her about being an accomplice if she doesn't tell the truth, and the fact that even if she saves her son's life, other children will lose their lives because of her.
Cardo tells Glen he plans to go to Punta Roa to gather information. Glen asks who he'll visit if he doesn't know whom Belinda visits. She wants to accompany him.
Ivan calls Fernan to ask what's wrong. Fernan replies he isn't sure whether they continue the procedure or not. Ivan is mad and demands if the police already know about them. He demands to talk to Tony and tells him to ensure they don't botch their job. After the phone call ends, Tony tells Fernan they still have some children to take, but the latter replies he has no plan to return to Tomas. Tony expresses that Tomas has an idea about their sideline, or that he has someone follow them. Joaquin is driving behind them, intent to follow them to Ivan's hideout.
The medic (Elsa) wake up Junior to eat. When she removes the blanket, Junior is nowhere insight. Junior has hidden behind a door and tries to leave, but Elsa grabs him. She kneels down and tells him that even if he escapes the room, there are a lot of men guarding outside,
Edgar and his family are in their dining room, praying for Junior's well-being. Norma prays for Carmen's clarity of mind to understand what's happening, especially when she isn't with them. Carmen is crying on her bed, clutching a pillow.
Crying, Junior tells Elsa that his mother is worrying and that he no longer has a father. He begs her to let him go. Elsa starts to feel compassion as Junior grasps his hand. She stands up, removes her face mask and tells Junior to stop crying. She holds Junior close to her. Unknown to them, Ivan has seen the entire spectacle (because she left the door open!)
Alone in Cardo's car, Glen is on her phone and apologizes her mother: she doesn't know what she'll go home. Lolit tells her that between a married couple, it's usually the husband who works overtime. Cardo sees Glen talking to someone over the phone. Nanding tells his daughter to be careful. He tells her to tell Cardo to keep her safe.
Norma is talking with Carmen over the phone. They will visit her tomorrow. Edgar asks his wife why Carmen doesn't want to return to their house. Edgar says he doesn't want Carmen to be with Joaquin and doesn't like him anymore. He questions why Joaquin is separating Junior from Lola Flora. Norma says because Carmen decided to marry Joaquin, she's simply standing by her choice.
Ivan is having a meal with Elsa. He asks her if Junior is eating and if he's giving her a hard time. He tells her that hard-headed kids deserve to be punished. Elsa defends Junior. Ivan notes she used the boy's first name and blatantly accuses her of showing sympathy/pity for the child. He tells her he doesn't like it when one of his team member feels pity for their children. His bodyguard restrain Elsa while Ivan opens a drawer and takes a syringe. He tells her it won't hurt and injects her on the nape.
As Joaquin continues his pursuit, Fernan notices a car trailing them from behind.
A gagged Elsa wakes up on the surgeon's table. Ivan grabs a pair of forceps and hushes her to be quiet because he can't focus--because he doesn't want to be charged with negligence. She begs him to let her go.
Junior is alone in his room, crying for Tito Cardo.
Ivan begins to hear voices tellin him he's not a failure. He counters it and talks out loud that he isn't.
Ivan tells Elsa she's healthy enough to donate her organs. He grabs the scalpel and hushes her, "This operation will be a success." However, Ivan beings to have a monologue with himself. In one minute, he's telling himself that he's a failure. In another minute, he tells himself he's not. He starts to poun dhis head.
He abandons the scalpel and grabs a syringe to extract a medicine from a bottle. As Elsa is screaming and crying for her life, he injects her on her right brachial arm. Suddenly, Elsa's body convulses. Dazed, he aks himself what he has done. He begins to tell himself he's a failure. Elsa stops convulsing and finally dies. Ivan stops his monologue and touches Elsa's face, laughing like a maniac.
Cardo tells Glen he can go alone. Glen says she will continue to stick with him. He tells her that her parents are right about her safety. Glen says that all parents are like that. Cardo tries to discourage her and tells that what they're doing is an unofficial business. Glen won't budge and tells Cardo there's no turning back since they have already started it, "tsaka, hindi mo naman ako pababayaan diba?"
Ivan's goons tell the other staff to eat faster and listen to what the Doctor says. "Let what happened to Elsa be a good reminder."
Cardo calls his Lolo Uncle that he plans to go back to the subdivision. Delfin asks if Cardo will inspect each house. Cardo tells his Lolo that he (Delfin) knew what Cardo would do and asks for access to the subdivision.
Fernan tells Tony to drive faster. Joaquin utters that they finally noticed them, "Tapusin na natin ang mga 'to."
Cardo tells Glen that Delfin isn't answering the phone. They suddenly hear gunshots.
Joaquin has swerved Fernan's car and fired shots at him. Greg and Tony is shot. Fernan manages to drive away. Dino tells Joaquin Greg is shot and drags him inside the car.
Tony tells Fernan he doesn't want to die. Fernan calls his friend, Emong, for help. Emong tells him to go to his hideout. As Fernan car drives past Cardo, Glen says it's the same car that killed her witness. They decide to follow it.
Fernan arrives in Emong's hideout and they go upstairs. Dino tells Joaquin to bring Greg to a hospital. Joaquin replies they'll do it later, unless Fernan escapes.
Cardo and Glen arrive at Emong's handout and grab their guns. They make their ascent to the stairs.
Emong, Fernan and Tony sees the pair and grabs their guns too. Cardo tells Glen to stay as he goes inside.
Fernan fires the first shot and a wave of gunshots ensure. Two men are making their way to an unsuspecting Glen. She eventually notices and fires shots at them. Glen decides to follow Cardo while shooting down some men.
Joaquin arrives at the hideout and spots Cardo's Jeepney. He sees Tony descend the stairs trying to escape.
Cardo continues to shoot several men, until Fernan appears and strikes him with an axe. Cardo manages to get a lid and defend himself as Fernan strikes him. After a series of parries and blows, Cardo overpowers him and nearly throws him outside the balcony. Cardo tighly grasps Fernan's hand and asks where the kids are. Fernan refuses to answer and tells Cardo to let him go. Fernan's hand eventually slips Cardo's grips and he falls several stories down. Cardo and Glen can only look at Fernan's body in shock.
Colonel Carreon asks Cardo if it was a coincidence that he was planning to visit Punta Roha. Cardo confesses he actually planned to go there. They discuss how there was no other witness who saw what happened in Emong's hideout.
Dino calls Joaquin and asks how Greg is doing. He adds, "Baka magsumbong si Tony tungkol sa negosyo niyo." Joaquin replies he will deal with him as soon as possible.
Cardo tells his Lolo uncle and Colonel Carreon that he won't stop until he finds Junior.

Commentary:
- looks like Dr. Ivan is suffering from auditory hallucination, or he's mentally ill. Yeah, definitely he's mentally ill.
-kawawa si Elsa, I liked how the director portrayed her resolve to help Junior. Sa previous episodes, lagi siyang may face mask para hindi marecognize ni Junior ang mukha niya. Pero nung nagmaka-awa si Junior sa kanya, inalis niya ang face mask at niyakap siya. Ibig sabihin nito, hindi siya takot marecognize ni Junior dahil willing siyang tulungan ang bata. Sayang nga lang pinatay siya ni Eric Quizon. Ginawa siyang plot device para ipakita sa viewers kung gaano siya kasama.
-Maganda rin ang motive ni Dr. Ivan for being evil. He believes he's doing a right thing because he's saving children's lives. But he doesn't realize that he's also kidnapping and killing innocent children just to save another child's life. Ito ang mga nakakatakot na tao. Naniniwala silang tama ang ginagawa nila, kahit nakakasakit na ang mga kilos nila. Dr. Ivan is "Evil" because he believes he can play God and save other children's lives.
